1. Same Day Assessment and Staffing

Because you care about getting yourself or your loved one help right away, HomeWell assigns you a dedicated Care manager to give you an in-home consultation the very same day you call. This 30 minute visit is free and there is never any obligation nor pressure to accept our service. We are there for information sharing purposes. At the end of your consultation, once you decide to move forward, your Care manager will initiate a comprehensive needs assessment designed to give you the best service at the lowest cost. Your HomeWell Personal Care Assistant can begin providing excellent care right away.

 

 

 

 

2. Individualized Care Plans

    

HomeWell works with each client to credate an individualized care plan that pays attention to the things that are important to you. Your dedicated HomeWell Care manager will take the time to get to know you to ensure that you are receiving the help you want and need. We do not suggest costly extras or inflexible packages. Your HomeWell personal Assistant completes your individual care plan checklists and conducts a brief satisfaction survey at the start and end of every shift to make sure that we consistently meet your care needs.

 

 

 

 

3. Life Enhancing Activities Program

5. Daily Checklist

    

Because you care about having all of your care needs met, your HomeWell Personal Assistant completes a daily detailed checklist that covers every aspect of your personalized HomeWell care plan. The chore and personal care checklist is completed, signed and verified by HomeWell and kept on file. Upon request. Copies can be sent to your for your review. Should you ever need to make a change to your HomeWell care plan, your dedicated Care manager is happy to assist you and to update the daily checklists. 6. Daily Satisfaction Survey

    

To consistently receive the excellent service you deserve, every HomeWell Personal Assistant asks three important questions at the beginning and near the end of every shift that ensure quality care and service every time. Asking the before and the end-of-shift questions allows your HomeWell Personal Assistant time to address any requests. Should you or your loved one ever has a request or concern, your Personal Assistant or dedicated HomeWell Care manager is there to help.

 

 

 

 

7. Safety and Security

    

To ensure safety and security for seniors in their homes, your HomeWell Care manager completes a safety checklist. Furthermore, we visit our clients regularly to reassess their needs.
